Area 01: Foundation

Placement Stability & Keywork

Placement stability is the foundation upon which all other outcomes are built. We assign a dedicated keyworker to every young person, establish structured weekly keywork sessions, and implement proactive disruption management strategies from day one.

Keyworkers build trusting, consistent relationships through regular one-to-one sessions focused on the young person's goals, wellbeing, and daily experience.

Measured Outcomes

  • Reduced unplanned endings and placement moves
  • Consistency in weekly keywork delivery
  • Early identification and intervention of risks
  • Transparent and regular progress reporting

Area 02: Life Skills

Independence & Daily Living Skills

We provide practical, structured support to develop the daily living skills young people need for successful independent living cooking, cleaning, budgeting, shopping, personal hygiene, and managing a tenancy.

Independence skills are assessed using a structured tool, with targets reviewed at every keywork session. Staff model and coach, encouraging growing confidence and competence.

Measured Outcomes

  • Significant improvement in skills assessment scores
  • Competence in independent meal preparation
  • Financial literacy and budgeting confidence
  • Full tenancy readiness prior to move-on

Education, Training & Employment

We actively support every young person to access and sustain engagement in education, training, or employment. This includes identifying suitable providers and advocating for the young person.

  • Reduction in NEET status within 8 weeks of placement
  • Active support with CV building and interviews
  • Consistent monthly tracking of EET progress

Area 04: Personal Growth

Personal Development & Social Inclusion

We support young people to build self-confidence, develop positive social networks, and engage meaningfully with their communities. This includes structured activities, mentoring, and community participation.

Staff facilitate access to youth groups, cultural events, sports clubs, and volunteering. Keywork sessions include goal-setting around identity, belonging, and personal development.

Measured Outcomes

  • Active participation in community engagement activities
  • Development of positive and supportive peer networks
  • Measurable growth in self-confidence and self-esteem

Health & Wellbeing

We support every young person to access appropriate healthcare and develop healthy lifestyle habits from GP registration in the first week to mental health support and healthy eating education.

  • Regular and recorded emotional wellbeing check-ins
  • Coordination of timely specialist health referrals
  • Engagement in healthy lifestyle and activity goals

Area 06: Move-On

Transition & Move-On Planning

We plan for move-on from the very start of every placement. Our goal is that every young person leaves with a planned, positive transition into appropriate independent or supported accommodation.

Keyworkers develop transition plans covering housing pathways, tenancy readiness, and ongoing support networks, liaising closely with housing providers and personal advisors.

Measured Outcomes

  • A clearly defined move-on plan for every resident
  • Regular tenancy readiness skills assessments
  • Clear housing pathways identified and actioned
  • Effective post-placement follow-up and transition
